Ritsuko refers to him by a shortened version of his given name, while Misato, who has been his lover, is more formal and always calls him by his surname. UrsusArctos: Ritsuko doesn't seem to mind the moments of friendship/flirtation with Kaji, but Misato uses uncharacteristic formality to push herself away from him - as it turns out by the end of this episode, not very successfully.
