However, she was involved in a "contact experiment" with Eva Unit 02 which trapped the part of her soul containing her maternal aspects inside of the Evangelion, driving her clinically insane in the process. She was institutionalized and began behaving as if a doll she possessed was really her daughter, and totally ignored Asuka.  Ultimately, Kyoko committed suicide by hanging herself and the doll-Asuka.

People attending Kyoko's funeral provide insinuations that Langley's infidelity was the ultimate driving force behind the suicide itself.  
In a flashback in ep. 25, we hear insane  Kyoko begging Asuka "Please die with me".
